Flight of Fancy New Airbus Jet Ends in Disaster
The inaugural U.S. flight of one of Airbus’s newest aircraft models was supposed to be a big deal, showcasing the impressive new jumbo jet on its very first trip from New York’s John F. Kennedy Airport.
Instead, it was spectacularly terrifying and embarrassing fail. The high tech jet’s computer system aborted it’s own takeoff — because it deemed the runway too short.
Journalists and staff were among the passengers on the Airbus A350 for the chartered Qatar Airways flight that was supposed to travel 12 hours to Hamad International Airport.
The airplane even had screens on each seatback via which the passengers could watch the taxi and takeoff as it happened. Unfortunately, rather than watching the plane soar to 30,000 feet, it taxed, picked up speed — and then came to a startling and screeching halt.
According to The Points Guy Editor in Chief, Zach Honig, who was one 36 journalists and staff members on the charter flight, “About 18 seconds after we began rolling down JFK’s runway 22R, the aircraft self-aborted, bringing us from more than 100 mph to a loud, screeching halt in roughly 15 seconds.
“For a plane of this size and weight, stopping that quickly required a lot of force.” 
Though no one was injured, passengers on the flight were confused, and some were scared and upset and wanted to get off.
The flight crew told passengers to remains seated. They also told them to turn off their cameras and Honig was also asked to stop tweeting updates for his 9,600 followers, according to the Daily Mail.
Eventually the highest-ranking executive on the Airbus explained to the travlers there was no reason to fear for their safety. “For some reason the A350 decided that our 11,000-foot runway was too short to support the takeoff, and the plane applied the brakes at full force — all on its own,” writes Honig.
The New York based flight, which had arrived earlier from Doha, was meant to give Qatar Airways the ability to boast the first flight Airbus 350 flight to and out of the U.S., “a significant milestone in what’s quickly becoming one of Qatar’s fastest-growing markets,” says Honig. It also gave the airline the chance to one-up Delta on its home turf — the airline will be launching flights on the Airbus 350 in 2017 — and to show the goods off to journalists.
After the failed takeoff, and after a nearly two-hour delay, (and more than a few drinks for the passengers) the Qatar Airways Airbus 350 did takeoff and fly to Doha.
The Airbus 350 is set to begin service on from the U.S. on Qatar Airways on Jan. 1, 2016.

3.4 Obstacle Control
 
Every aerodrome operator should establish Obstacle Limitation Surfaces (OLS) in accordance with
ICAO SARPs for safe take-off and landing of aircraft. Objects other than approved visual and navigational aids should not be located within the obstacle restriction area or the aerodrome without specific approval of each state’s CAA. If there are any objects penetrating these surfaces, the aerodrome operator should remove or mark and/or light them.
 
3.4.1 Establishment of OLS (Obstacle Limitation Surface)
 
 
Establishment of obstacle limitation surfaces at each aerodrome depends on types of instrument
runways, either non-precision approach or precision approach runways. Established obstacle control
surfaces must be prevented from becoming unusable by the growth of obstacles. Obstacles range
from fixed - (temporary or permanent), to mobile objects, or parts thereof which penetrate the obstacle limitation surfaces.
 
The following OLS should be established for anon-instrument runway and non-precision instrument
runway:
 
Conical Surface;
 
• Inner Horizontal Surface;
 
• Approach Surface;
 
• Transitional Surface; and
 
• Take-off Climb Surface.
 
 
The following OLS should be established for a precision instrument runway:
 
• Outer Horizontal Surface (as may be defined by the state);
 
 
• Conical Surface;
 
• Inner Horizontal Surface;
 
• Approach Surface;
 
• Inner Approach Surface;
 
• Transitional Surface;
 
• Inner Transitional Surface;
 
• Balked Landing Surface; and
 
• Take-off Climb Surface.
 
 
Where two OLS surfaces overlap, the lower surface must be used as the controlling OLS. Detailed information about ‘Dimensions and slopes of obstacle limitation surfaces’ can be found in ICAO Annex 14,
 Height of the obstacle in relation to the aerodrome elevation and   
The period for which it will remain an obstacle (if it is temporary obstacle)
 
Temporary obstacles and transient (mobile) obstacles, such as road vehicles, rail carriages or ships, in close proximity to the aerodrome and which penetrate the OLS for a short duration should be referred to the CAA to determine whether they will be a hazard to aircraft operation. Fences and levee banks that penetrate the OLS should be treated as obstacles. Obstacle surveys of areas underlying the obstacle control surfaces at both proposed and existing airports are performed to identify the location and height of objects that may constitute infringements of these surfaces. Survey specifications are used to obtain the obstacle data necessary to comply with the ICAO SARPs contained in the relevant Annexes and the requirements of each State’s regulations. Obstacles must be designated: for example, tree, hill, pole, tower, spire, vent, chimney, mast, post, antenna, building, house, etc.
 
3.4.2 Procedures for Aerodrome Operators to Control OLS
Aerodrome operators must establish procedures to monitor the OLS and the critical obstacles associated with aircraft operation and have them included in the aerodrome manual. Each aerodrome operator should monitor the OLS applicable to the aerodrome and report to its civil aviation authority any infringement or potential infringement of the OLS. When a new obstacle is detected, the aerodrome operator should disseminate the information to pilots through a NOTAM, in accordance with aerodrome reporting procedures set out in Chapter 3.2. Information on any new obstacle should include:
• The nature of the obstacle – for instance, structure or machinery;
 
• Distance and bearing of the obstacle from the start of the take-off of the runway, if the obstacle is within the take-off area, or the approach area; 
 
3.4.3 Type A Charts
A Type A chart is a chart which identifies information on all significant obstacles within the take-off area of an aerodrome up to 10km from the end of the runway. A Type A chart should be prepared for each runway that is used in international operations. Obstacle data to be collected and the manner of presentation of the Type A chart should be in accordance with the standards and procedures set out in ICAO Annex 4. An electronic chart may replace a paper chart. Where a Type A chart has been prepared and issued the take-off flight area should be monitored and any changes to the Type A chart information must immediately be communicated to all users of the Type A chart. Obstacles may be recorded in an electronic Terrain and Obstacle database (e-TOD).

Improving African Aviation Safety in 100th Year of Commercial Aviation


Aviation in Africa looks bright And the African youth can make it Happen  
2014 marks 100 years of commercial air travel. On January 1, 1913, a single passenger crossed Tampa Bay in the United States on the world’s first regular scheduled flight service. Since that landmark, air transport has grown from one passenger on one route to 3.3 billion passengers on more than 40,000 routes. Air connectivity has transformed people’s lives, widening their horizons, culturally,socially and economically.

This year the African Union marks its 50th year and air transport has never been more relevant to the organization’s aim to promote African integration. For a vast continent with challenging natural barriers to ground transportation, aviation is crucial for connectivity and development. Moreover, aviation supports 6.7 million jobs on the continent, with a significant proportion of those in high-tech and advanced skills work.

In monetary terms, the industry stimulates a $67.8 billion contribution to Africa’s GDP. Aviation facilitates tourism and business services, while air freight is crucial for the success of African organic produce, and supporting a growing manufacturing base.

African governments have much to gain from pursuing growth and connectivity. Improved safety is absolutely central to that goal. Safety is clearly uppermost in our minds at the moment. Our thoughts are with the families and friends of all on board flight MH370.

This tragedy reminds us that safety is a constant challenge everywhere. And every accident and every fatality in aviation re-dedicates the entire industry to making flying even safer.

One way we do this is to investigate the accident thoroughly and from the conclusions drawn, apply any recommendations. In parallel, we must ensure that the global standards and best practices which already exist are applied and implemented.

This is a challenge for Africa. As of the end of 2013, only 11 African states had achieved 60% implementation of the International Civil Aviation Organisation (ICAO) safety-related standards and recommended practices. That is why aviation stakeholders including IATA, ICAO and others have united behind the Africa Strategic Improvement Action Plan aimed at addressing safety deficiencies and strengthening regulatory oversight capabilities. This action plan was endorsed by States of the African Union in the Abuja Declaration of 2012.

The Abuja Declaration has a target to bring the African accident rate in line with the global average by 2015, and the action plan commits States to:

Ø Adopt and implement an effective and transparent regulatory oversight system. This includes mandating the implementation of the IATA Operational Safety Audit (IOSA) to all airlines

Ø Implement runway safety measure
Ø Training on preventing loss of control
Ø Implement flight data analysis (FDA)
Ø Implement Safety Management Systems (SMS)
Ø Operation Health and Safety Management System (OHSAS)


Evidence already shows that applying global standards can make a difference in African aviation. There has been progress in recent years. The Western-built jet hull loss rate improved 55.4% between 2012 and 2013, while the region’s accident rate for all aircraft types improved nearly 50% (7.45 accidents per million flights from 14.80 in 2012).But despite this, it remains a fact that on average in 2013, there was one accident for every 493,000 flights on Western-built jets in Africa. That was almost five times the global average. And we cannot take the recent improvement trend for granted.

To make these gains a sustainable foundation on which to achieve world-class safety levels is going to require the continued determination and commitment of all stakeholders, including governments. Global standards are the key. Governments must effectively regulate them. And airlines need to operate using them.

An example of a global standard which airlines can implement is the IATA Operational Safety Audit (IOSA). IOSA is a requirement for all IATA member airlines, but it is an open program, not an exclusive club. Of the 390 airlines on the IOSA registry at the end of 2013 about a third are not IATA members, and together their safety performance was more than twice as good as those carriers not on the registry.

African airlines on the IOSA registry are performing almost seven times better than non-IOSA operators in the region, and the International Airline Training Fund commenced a program to assist ten airlines who wished to prepare for an IOSA inspection. Of these, three are now on the IOSA registry and a fourth is expected shortly.

This year, we will extend this Implementation Training Initiative to a further 10 eligible airlines in Africa. And we are also beta testing a Basic Safety Audit Program. This is not an IOSA-light, but it can be used by airlines whose nature or scale of operations is outside of the IOSA scope.

Over the years there have been many initiatives to improve African safety. While progress has been made, the problem has not been solved. This time could be different. The eyes of the world are on the continent’s economic expansion. It is a great opportunity to co-opt that into support—technical and financial—for connectivity that underpins economic ties.

" But that will only happen if there is a solid commitment on the part of African governments at the highest levels. Everyone knows what needs to be done. Now we need tangible signs from African governments for a strong and comprehensive follow-up program. Besides the aviation professional and enthusiastic African youth can be of immense help ". As I foresee and believe in the African youth and their perseverance to get Africa reach the highest category in Aviation Safety.

The aviation industry is determined and eager to work with all governments and stakeholders in Africa to deliver the highest safety standards. 100 years after the first passenger flight, African aviation stands poised for an amazing decade of growth and opportunity – let us be sure this chance is not missed.
